At Axis Physical Therapy, we evaluate and treat a wide variety of injuries and conditions. Our goal is to restore and promote optimal physical function for each patient. After a thorough initial evaluation, we will identify potential and existing problems. Our therapists will discuss the findings with you and outline their treatment plan. We specialize in providing hands-on care utilizing a variety of manual therapy and soft tissue techniques, therapeutic exercises as well as using modalities as needed. Your initial evaluation, as well as follow-up visits, generally will last for 45-60 minutes.
Our office forms can be downloaded and completed prior to your visit.
Should you prefer to complete them at our office, please arrive approximately 10-15 minutes in advance of your appointment time. We would like to suggest wearing loose and comfortable clothing for your initial evaluation.
We will provide you with a personalized home exercise program within the first 1-2 visits and periodically update the program.
